Before scheduling a ride, you must register with Stateline Mass Transit.  Please complete the following application and fax to 815-961-9892. Please allow 48 hours for your application to be processed before scheduling your first ride.  Applications may also be mailed to:  520 Mulberry Street, Rockford, IL  61101.
